1. Notification of Death Claim by the banks
2. Death Certificate certified by the bank officers
3. Original copy of MRTA certificate
4. Copy of NRIC of deceased
5. Claimant's statement (could be obtained from the bank or the insurance company)
6. Doctor's statement
7. Police report (if applicable)
